A. Did you achieve your project’s objectives? Please reference the objectives
you listed on your application.

B. How did you measure your success? Quantitative assessments are good means of evaluating project effectiveness. Examples might include grades, SOL scores, pre- and post-testing, written or other portfolio work, teacher/student/parent surveys, teacher journals, school accreditation, evidence of teacher training, evidence of use by teachers, etc.

C. Was this a valuable opportunity for your students to experience something truly unique? Why and what was it? Give human-interest examples of how your grant inspired students, gave them opportunities for new learning, and/or offered them experiences they would not otherwise have had. Specific quotations from students, photographs of the learning in progress, etc. are encouraged.

D. What feedback did you seek and receive from your students, peers, administration, parents and community? Give examples of success related to project cooperation among staff in your building or collaboration with parents, other people, and/or organizations in the community.

E. Is there potential for duplication of this project at your (or another) school or
in subsequent years (include potential carry-over of materials or
equipment)? Did you make modifications to your project as it developed?
